Share your story of hope

I acknowledge by submitting my family story I hereby grant the TSC Alliance permission to use my family's story and my/my child's photo of which I have submitted without payment or other considerations to be used in any and all of its publications, including print or web-based publications, website and/or social media channels without notifying me. The TSC Alliance reserves the right to choose when and if to utilize a story of hope. I understand that once my image is posted online, the image can be downloaded by any computer user. Therefore, I agree to indemnify and hold harmless the TSC Alliance, its agents, volunteers, and employees from any claims. I irrevocably authorize the TSC Alliance to copy, edit, enhance, crop, or otherwise alter any photo/video/digital media for use in their materials. I also waive any rights for approval or inspection of any photo/video/digital media, including written or electronic copy, wherein my story or likeness appears. Additionally, I waive any right to royalties or other compensation arising from or related to the use of my story and photograph. I hereby hold harmless and release and forever discharge the TSC Alliance from all claims, demands and causes of action which I, my heirs, representatives, executors, administrators or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization. I have read this release before acknowledging below and I fully understand the contents, meaning and impact of this release. I understand that checking the box below and typing my name is equivalent to my signature.
